Africa: AIB says Suntai was not qualified, certified to fly crashed Cessna 208B Caravan aircraft

ACCIDENT Investigation Bureau ( AIB) today released the report of the Cessna 208B Caravan aircraft that cost the life of former Taraba State Governor, late Danbaba Suntai and three other passengers revealing he was not certified, qualified or competent to fly the aircraft.

This is just as the Bureau revealed in its report that it could not conclusively determine the cause of this accident which occurred at Kwanan-Waya Village, Yola South Local Government Area, Adamawa State On 25th October 2012, seven years ago.

AIB made this known today as it released the final report of six occurrences including the report on the accident involving a Bristow Helicopters (Nigeria) Limited Sikorsky S76C++ Helicopter with nationality and registration marks 5N-BQJ, report on the accident involving a Boeing MD-83 aircraft operated by Dana Airlines Ltd with Nationality and Registration Marks 5N-SRI, report on the Serious Incident involving an Airbus A330-223 aircraft operated by Delta Air Lines Inc. with nationality and registration marks N858NW.

Others are report on the accident involving a Cessna 208B Caravan with Nationality and Registration Marks 5N-BMJ belonging to Ministry of Works and Transport, Taraba State Government, report on the accident involving a Diamond DA40D Aircraft operated by International Aviation College (IAC) Ilorin with nationality and registration Marks 5N-BRD, and report on the serious incident involving Gulfstream G200 Aircraft Operated by Nestoil PLC.

According to the final report released on the Cessna 208B Caravan with registration marks 5N-BMJ belonging to Ministry of Works and Transport, Taraba State Government, flown by the late former governor, Suntai was qualified to fly a Cessna 172 but on the day of the unfortunate accident he commandeered a Cessna Caravan 208B.

The AIB also identified the decision of the pilot to operate a Visual Flight Rules (VFR) flight after sunset when he was not type rated on such operations as well as inadequate oversight by the Regulatory Authority as contributory causal factors.

Also that the pilot does not have instrument ratings and night flight privileges and reported an incorrect estimated time of arrival (ETA at Yola as 10:01 UTC as against the time 17:19 UTC.

He also reported the number of persons on board as 06 to the Control Tower as against 04 actual persons found after the accident.

The AIB in it safety recommendations called on the Nigeria Airspace Management Agency ( NAMA) to take appropriate action to relocate the existing control tower at Yola Airport in order to enhance the aerial view of the approach path of runway 35 from the Tower.

The Bureau also advised NCAA to ensure all pertinent regulations with regards to the operations of the aircraft and certification of all relevant personnel and facilities of the Ministry of Works and Transport, Taraba State Government are appropriately complied with.

    The author of this piece is not an accomplished writer and knows less than ZERO about aircraft and aviation.

    When the AIB releases a formal report regarding the ’cause of a fatal airplane crash’ – one would think that cause would be quoted in the body of the story. But no…irrelevant ‘gibberish’ was recited with no relationship to the actual CAUSE.

    Flying at night, flying under VFR regulations, inadequate oversight by the CAA, not being type rated in that airframe…and NONE of the other anecdotal comments…can be the actual description of the events leading up to the crash.

    Obviously, the plane impacted the ground..and did ALL the occupants die? Why did the airplane impact the ground? Was it inverted? Did the pilot lose control? Did it run out of fuel? Was the plane overweigh? Was someone – other than the pilot – at the controls when it crashed?

    The entire story – as written – is pure nonsense. The piece was poorly written, significant details were SKIPPED, other ‘irrelevant’ inclusions of OTHER crash investigations detracted from the headline – and it left numerous ‘unanswered’ questions in the readers’ mind.

    To be sure – the people who staff the AIB – are NOT the most capable, most analytical nor most educated crash investigators. There are no such individuals anywhere in Nigeria. The AIB is not capable of determining the FACTUAL cause of an airplane crash. They don’t have the tools, the intellect nor the authority to achieve purely objective and complete crash data.

    Apparently, no one will ever know what happened to the Caravan.
